Participant Forms

Please complete the appropriate form before your scheduled session, ceremony, or group experience with The 4th Rose. These forms help support informed consent, participant safety, and clear communication before we begin.

Select the form that corresponds with your experience below. Each form can be completed and signed electronically, and a copy of your responses will be emailed to you after submission.

Individual Sessions

For private one-on-one sessions with The 4th Rose, including Reiki, private sound healing, and Reiki + sound sessions.

Group Experiences

For group offerings with The 4th Rose, including sound baths, meditation, breathwork, yoga, somatic movement, and signature immersive experiences such as The Embodied Feminine and Unfold.

Cacao Ceremonies

For cacao ceremonies with The 4th Rose where ceremonial cacao is offered as part of the experience.
